Just a few of the questions in the FL DOH Survey. It was given to 1,650 middle and high school students in the SDIRC in 2021.
Did you sign a parental consent form for this? The district claims they only gave surveys with parental consent. If you DID sign for the health survey...did you know these questions were in there starting at number 48 of the survey? This survey was given to students ages 10 to 17.
Parents, grandparents, and community members please don’t sign anything for your children or grandchildren without knowing what you are signing! See screenshot of public records request for this survey below. Our children deserve better and this has NOTHING to do with education!
